We've been serving families and individuals with the most advanced, comprehensive clinical home care in NJ since 1912. -We are currently looking for -a Physical Therapist -to join our team. -The position would be responsible for - providing physical therapy evaluation and treatment to patients in their homes or alternate community settings.

• Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersAssesses and evaluates client for physical therapy needs and develops and implements appropriate plan of action in collaboration with the physicianDocuments all changes in client's condition and updates care plans and outcomes accordinglyIdentifies counseling needs and plans appropriate teaching strategiesMonitors quality and appropriateness of services providedAdheres to agency infection control and safety policiesAdheres to the organization's policy in regards to absenteeism and appearance
