We are innovating value-based care and leveraging integrated applications, population insights and staffing expertise to ensure all patients have access to high-quality, connected care services that provide better outcomes at a reduced total cost of care.Please Note: We will only contact candidates regarding your applications from one of the following domains: @summithealth.com, @citymd.net, @villagemd.com, @villagemedical.com, @westmedgroup.com, @starlingphysicians.com, or @bmctotalcare.com.Job DescriptionEssential Job Functions and Responsibilities:Performing and documenting initial evaluations and follow-up treatment visits in accordance with state practice act and consistent with current best-practices.Establishing appropriate plan of care for patients including prognosis, short/long term goals, and complete documentation in a timely manner.Prioritize a patient centered approach with a focus on patients desired goals and function.Regularly reassess patient progress and modify plan of care as necessary.Communicate effectively with clinic staff, physicians, and other members of the patients healthcare teamProviding supervision for Physical Therapist Assistants, ancillary staff, students and volunteers as requiredMaintaining a full patient caseload and remaining flexible to accommodate patients. Follow-up treatment sessions are scheduled for 30 minutes one-on-one with each patient.Requirements include:Doctorate Degree required2-4 years of experience required; 5-8 years' preferredNew graduates may apply if strong experience in outpatient settingCurrent NJ Physical Therapy LicenseBLS CertificationAbility to work independently or as part of a team required.Advanced certification (OCS, CSCS, COMT, MDT, etc or similar) preferred but not required#INDAncillaryAbout Our CommitmentTotal Rewards at VillageMDOur team members are essential to our mission to reshape healthcare through the power of connection.
