Position Highlights:
Collegial multispecialty group practice with a large referral
base
Focus on team-based care

Advanced Practice Clinician Partnership Plan and unlimited room for

Primary Responsibilities:
Perform comprehensive and problem-focused medical history and
physical examinations
Develop and implement treatment plans for patients assigned within
the specialty care team
Develop and actualize plans for the specialty care team as
assigned
Provide guidance to nursing personnel towards treatment plans and
care issues
Document treatment plans in the medical record
Collaborate with physician and documents physician guidance as
needed
Coordinate care for patients

-Required Qualifications:
Advanced Nurse Practitioner Certification or Physician Assistant
certification
Active unrestricted APRN or PA-C license in New York, or ability to
obtain prior to start -
Active and unrestricted DEA License or ability to obtain prior to
start -
Experience in primary care or endocrinology -
The salary range for this role is -$104,500 to $156,000 annually
based on full-time employment. Add the Additional reference to
account for variable pay if Sales (84-92) or Physicians (M2-M4).
Pay is based on several factors including but not limited to local
labor markets, education, work experience, certifications, etc.
