No on-call or weekend hours required!Position Title: Registered Nurse (RN) Care ManagerDepartment: Care ManagementReports To: Care Management Nursing SupervisorEmployment Type: Full-Time, ExemptWork Location: Clinic-based and community settings (North Philadelphia)Position SummaryThe RN Care Manager provides holistic care management aimed at improving health outcomes and reducing ER visits and hospitalizations among high-risk patient populations. This role collaborates with PCPs, Community Health Workers, and other care team members to implement individualized care plans, deliver health education, and coordinate access to medical and social services.Key Responsibilities

• Develop and implement care plans with PCPs for chronic disease management
• Collaborate with interdisciplinary team members (dietitians, social workers, CHWs, etc.)
• Provide patient education on disease processes and general health maintenance
• Conduct medication reconciliation, phone check-ins, and home visits as needed
• Support hospital discharge follow-ups and occasional in-clinic visits alongside providers
• Track preventive health maintenance and monitor care plan progress
• Conduct functional assessments and support Chronic Care Management (CCM)
• Document interventions, assess outreach needs, and adjust care plans accordingly
• Identify and help resolve care coordination inefficiencies
• Participate in outreach to high-risk populations with care gaps
• Integrate spiritual care in clinical practice as appropriate
• Travel to multiple sites as needed-no on-call or weekend work requiredRequired Qualifications
  • Current Registered Nurse (RN) license; BSN preferred
  • Bilingual in English and Spanish
  • 1-2 years of clinical experience required
  • Community health or outpatient case management experience preferred
  • Passion for serving low-income and underserved urban communities
  • Familiarity with North Philadelphia is a plus
  • Valid driver's license and insured vehicleCompetencies & Skills

    • Able to prioritize and manage multiple clinical and administrative tasks
    • Comfortable working independently in varied home and community environments

      • Work takes place both in health center clinical settings and in patient homes
      • Must be able to travel between EHC sites across North Philadelphia
      • Flexibility required for working in diverse home environments
