Job Description:
Carnegie Post Acute Skilled Nursing Facility is seeking a
compassionate and proficient Licensed Practical Nurse (LPN) to join
our dedicated healthcare team. In this role, you will be integral
in providing high-quality care and support to our residents, who
require post-hospital and post-surgical care. Our facility is
committed to enhancing patient well-being through exceptional
rehabilitation services, skilled nursing care, and a
patient-centered approach to health management. As an LPN at
Carnegie, you will work closely with RNs and other healthcare
professionals to ensure the delivery of optimal care tailored to
the individual needs of our clients. Monitor patient health and
maintain accurate health records. Administer medications and
treatments as prescribed by the attending healthcare provider.
Assist with patient mobility and personal care. Collaborate with
the healthcare team to create and implement personalized care
plans. Educate patients and their families on treatment and care
plans. Ensure compliance with all health care regulations.
Proficient in nursing practices and care Strong interpersonal and
communication skills Ability to administer medications accurately
Basic computer skills for updating patient health records Medical
Insurance Dental Insurance Vision Insurance Current LPN license in
the state of employment CPR certified IV certification
preferred
