Job Description Job Description Job description We are seeking a
licensed Physician Assistant or Nurse Practitioner to provide
comprehensive primary care services within a collaborative,
team-based practice. This role focuses on preventive care, wellness
visits, acute illness evaluation, and chronic disease management
for an established patient population. The Advanced Practice
Provider practices independently in accordance with New York State
regulations and organizational protocols, exercising sound clinical
judgment within scope of licensure. This position is exclusively
primary care and does not include specialty or surgical services.
Qualifications: PA or NP degree from an accredited program Board
certified in Family or Internal Medicine Active, unrestricted New
York State license DEA license (or ability to obtain) BLS/ACLS
certification Clean malpractice history Compensation & Benefits:
$120,000–$130,000 base salary plus up to $5,000
productivity/quality bonus Flexible 4- or 5-day workweek CME
allowance ($1,500) and 3 CME days PTO, paid federal holidays,
401(k) with match Health, dental, vision, life, and malpractice
