Jersey's premier healthcare system. -The Certified Mediciaion
Assistant administers medication to residents as delegated by the
Registered Nurse in accordance to facility policy. Provides direct
resident care to ensure resident's plan of care is followed.
Completes all required documentation as per state/federal
guidelines. Follows all facility policies and procedures.
A day in the life of a Certified Medication Assistant at Hackensack
Meridian Health's Nursing and Rehab facilities include: -Delivery
of Care: Demonstrates ability to provide appropriate resident care.
Provides resident care as per Resident Care Plan. Includes the
following tasks: ROM exercises, Ambulation, Transfer, ADLS,
Assisting with meals and feeding, Bed making, Personal Hygiene,
Skin Checks, AM and HS care, Vital signs, Answer pendants promptly,
Postmortem care, Assist with room changes, and Participate in
reports. Administer medication to residents as delegated by the RN
/ DON in accordance to facility policy. Documents administration of
medications on the Medication Administration Record. Reports any
issues to nurse in charge. Reports any refusal of medication or
observes side effects or adverse effects to the medication to RN /
DON. Ensures that all assigned residents are clean, neatly dressed
and groomed, safe, and fed during assigned shift hours. Reports any
unusual changes in resident condition to Charge Nurse. Communicates
pertinent information and actual or potential problems to charge
nurse throughout the shift. Completes all required documentation
accurately and timely as per facility/state/federal policies.
Monitor meal / room service, Vitals signs recorded as assigned.
Ensures residents are wearing/using adaptive equipment as ordered.
Participates in resident conferences and contributes to Service
Care Plan of a resident as requested. Encourages residents to
socialize and participate in activities. Additional duties as
Education, Knowledge, Skills and Abilities Required: Computer
skills, including Microsoft Office High school diploma or
equivalent Standing, walking and/or continuously moving around
Kneeling, stooping, crouching, crawling or working in other unusual
physical positions Climbing (including ladders, stepping stools,
and/or stairs) or balancing Stretching and/or reaching to
retrieve/replace objects Pushing, pulling, lifting and/or carrying
heavy objects (20 lbs. or more) without aid of physical devices
Dexterity and eye-hand coordination to perform activities which
involve holding, positioning, manipulating, typing (keyboarding or
data entry), etc. Visual ability to monitor instruments and
computers, distinguish characters (letters, numbers, etc.)
recognizes colors, observe physical characteristics, etc. Hearing
ability to recognize sounds, sound patterns and/or other auditory
stimuli Physical ability to react/respond quickly in emergencies,
including life threatening Education, Knowledge, Skills and
Abilities Preferred: Interpersonal and customer service skills
Time-management skills Multitasking and organizational skills
Previous nursing experience preferred Licenses and Certifications
Required: CNA or CHHA and CMA
