Job Description:
Summary of Job Function: Under the direction of the VP and
supervision of the Director of the Community Specific Program, the
Bilingual Patient Navigator will work with the multidisciplinary
healthcare team to ensure smooth planning and execution of outreach
activities. Will participate in institutional activities, attend
community events, track the project status, analyze and abstract
the data, perform data outcome analysis, and provide
report/presentation to support the management needs.
Job Requirements: Bachelor degree required. Must be fluent in
Chinese and English Bi-lingual, and able to read, write and speak
Chinese proficiently required. Experience in a health care setting
highly desirable
